Cutting holes into it will be fine, but you may want to seal the edges with some fabric tac or fray away to prevent any runs from appearing if the suit is low quality fabric.

There is no “fly” or zipper on the front, the only way in and out is the zipper on the back. So, you’ll have to unzip from the top of your head down to your lower back and roll it down to get to your plumbing.
